概述
positive predictive value是指阳性预测值,简写为PPV,是筛检试验检出的全部阳性例数中真正阳性症的比例,是一种反应试验结果阳性者患目标疾病的可能性。与阳性预测值相对应的是阴性预测值,这两者与灵敏度和特异性又有关。在进行一项诊断实验时判断是否患病时,结果分为四种:a,真阳性(实际有病,通过筛查试验确实判断为有病);b,假阳性(实际没病,但筛查试验判断为有病);c,真阴性(实际没病,筛查试验也判断为没病);d,假阴性(实际有病,筛查试验结果判断为没病)。阳性预测值=a/(a+b),阴性预测值=c/(c+d)。举例介绍:
一项诊断试验有100位受试者,其中10人有病,90人没病。经诊断试验检查,18人结果是阳性,82人结果是阴性,具体如下图。
在该诊断试验中,18人结果是阳性,但是真正患病的人只有8人,根据阳性预测值公式:阳性预测值=真正患病的人/诊断结果阳性的人=8/(8+10)×100%=44.4%。在该诊断试验中,共82人的结果是阴性,其中真正阴性的80人,则阴性预测值=真正不患病的人/诊断结果是阴性的人=80/(80+2)×100%=97.6%。根据这里的定义以及计算公式,可以看出阳性预测值如果越高,则说明该诊断试验的检出与患病结果越相符合,但是在实际的应用中,有的情况下不能选择最高的PPV的检测,因为需要权衡各个相关指标。
阳性预测值与灵敏度和特异性的关系
灵敏度=真阳性人数/(真阳性人数+假阴性人数)*100%;特异性==真阴性人数/(真阴性人数+假阳性人数)*100%。以一项100例受试者的诊断试验为例,该试验人群患病率为20%,即有20人患病,80人没病。灵敏度是50%,特异性也是50%,则PPV=10/50=20%。
如果将灵敏度提高到90%,则PPV=18/58=31%。
如果将特异性提高到90%,则PPV=10/18=55.6%。
根据上述的计算可以看出,灵敏度和特异性提高都会使PPV提高,但是特异性带来的效果更大。而在实际应用中灵敏度和特异性是一个相互对立的选择,我们无法即选择最高的灵敏度也选择最高的特异性,所以也导致PPV在很多时候不能选择最高值。必须要几个指标综合考虑,选择整体效果最好的结果才可以。

The synthetic challenge is mainly concerned with monomer synthesis, but the final polymerization sequence can be hard to control, leading to variable molecular weights and polydispersities and some degree of inconsistency in the properties of the same material between different synthetic batches. If a champion characteristic persists between samples, then the variation of other properties between batches can be tolerable, depending on the target application. In the case of polymers, we have chosen to study PPV-type polymers with bulky side groups that provide protection of their conjugated backbone from π−π stacking interactions. These polymers exhibit high photoluminescence quantum yields (PLQYs) in films and short radiative lifetimes and are an important benchmark to monodisperse star-shaped systems in terms of different absorption/emission regions. This Account therefore outlines the advantages and special features of monodisperse star-shaped macromolecules for photonic applications but also considers the two alternative classes of materials and highlights the pros and cons of each class of conjugated structure.
