Cytology is commonly used as a companion test to provide clinical resolution for equivocal or false-negative cystoscopy. Urine cytology can have high specificity but very low sensitivity; thus, cases of bladder cancer (including high-grade and muscle-invasive disease) may be missed due to false-negative results. Therefore, any advantage of high specificity is significantly reduced by low sensitivity. A clinically effective test to rule out UC requires high sensitivity and a high negative predictive value (NPV) with the clinical goal to have no, or very few, false negatives in the rule-out fraction. The diagnostic value of cytology is highest when it is performed by experienced cytopathologists, with local cytopathology showing considerable interobserver variability.
