概述
作用与用途
药效学:本药是芦丁经羟乙基化半合成水溶性黄酮类化合物,对闭塞性血管病具有较好的疗效。
药理作用:①抑制血小板聚集,防止血栓形成。②对抗5-羟色胺、缓激肽引起的血管损伤,增加毛细血管抵抗力,降低毛细血管通透性,从而防止血管通透性增高引起的水肿。③增加血氧含量和氧饱和度,促进新血管生成以增进侧支循环,对急性缺血性脑损伤有显著的保护作用。④可兴奋神经纤维,具有解痉作用。⑤还具有抗放射线损伤、抗炎症、抗过敏、抗溃疡等作用。
药动学:口服吸收良好,给药后1~6小时达血药浓度峰值。药物体内分布广泛,可通过血-脑脊液屏障。血浆蛋白结合率约为30%。在肝脏代谢,可能存在肠肝循环。血中消除半衰期为10-25小时。代谢产物70%从粪便排出。
临床应用:用于缺血性脑血管病及其所致的偏瘫、失语等,还可用于心肌梗死前综合征、中心性视网膜炎、血栓性静脉炎、静脉曲张等;也用于毛细血管通透性增加引起的水肿、淋巴回流受阻引起的淋巴水肿。
注意事项
不良反应:偶见胃肠道反应、过敏反应。有报道,静脉滴注给药后偶可出血心血管系统反应(如心律失常等)、肝脏毒性反应、急性脑水肿。用药期间避免阳光直射、高温及站立过久。如出现过敏反应,应立即停药。

Gentamycin is an aminoglycoside antibiotic that is widely employed for controlling Gram negative bacterial infections including that caused by the antibiotic-resistant Pseudomonas species. The clinical use of gentamycin is substantially limited by its side effects particularly acute kidney injury (AKI). The aim of the current study was to investigate the protective effect of the plant flavonoid troxerutin (150 mg kg-1 day-1 for 15 days) against gentamycin-induced AKI using Wistar rats as an experimental mammalian model. The results of the present work revealed that troxerutin significantly improved renal function as demonstrated by the increase in the glomerular filtration rate and the decrease in the levels of urinary albumin, urinary albumin to creatinine ratio, serum creatinine, and blood urea nitrogen (p < 0.001). In addition, troxerutin significantly attenuated gentamycin-induced renal tissue injury as indicated by the decreased protein expression of the renal tubular injury marker KIM-1, the attenuation of the renal histopathological changes, and the modulation of the oxidative stress markers as reflected by the decrease in the levels of lipid and protein oxidative modifications and the increase in the levels of reduced glutathione and total antioxidant capacity (p < 0.001).
