Organ preservation is critical for graft function. Small bowel transplants are particularly vulnerable to preservation injury, since intestinal epithelial cells are especially prone to ischemic damage. The interruption in oxygen delivery after procurement may lead not only to cell dysfunction and/or cell death but also to a collapse of the entire mucosal barrier function. Subsequently, bacterial translocation1,2 and septic episodes leading to critical illness3 not only threaten the graft but also the life of the recipient.4,5 Thus, the duration of ischemia from organ harvesting to implantation is critical for the intestinal transplant, rendering cold storage times greater than 8 hours more likely to induce significant damage to both the graft and the recipient.
