This patient was particularly noteworthy for several reasons. First, she had a previous history of "ALF", though it was extremely unusual in that she was taken to the operating room, but not transplanted. Secondly, she presented with an identical clinical syndrome during pregnancy during the current admission. Pregnancy associated liver diseases were particularly important to consider in this patient, but none of these fit the clinical picture. With her alteration in mental status, there was concern about the possibility of AFLP, but this typically presents later in pregnancy and the laboratory data did not fi this diagnosis. Additionally, her platelets were normal, making HELLP unlikely. Given that she had a similar episode in the past, there was concern about possible recurrent seronegative autoimmune hepatitis, making a liver biopsy important diagnostically. Liver histology excluded significant liver injury, and helped point to a diagnosis of a UCD.
