Transcatheter aortic valve replacement (TAVR) is indicated in patients with severe aortic stenosis (AS) who are deemed inoperable or are at intermediate or high surgical risk. Bicuspid aortic valve disease affects 1% to 2% of the U.S. population, and most of the affected patients require AVR at various ages. Bicuspid AS has been considered an anatomic challenge for TAVR procedures. Bicuspid AS has unique morphology compared with tricuspid AS,with large asymmetric heavily calcified leaflets, leading to a potential higher risk for paravalvular leakage or imprecise positioning during expansion of TAVR. Randomized trials have excluded bicuspid aortic valve morphology due to its unique anatomy that may negatively impact TAVR outcomes.
