In the most recent era, the vast majority of children who survive at least one year after HT can expect to enjoy excellent FS, meaning they are highly active without significant limitations. By contrast, children with end-stage heart failure have poor FS, a state that can be improved substantially by HT. Patients that are older, African American, have rejection during the first year after HT, are sicker or hospitalized at HT, and those receiving steroids at HT, are more likely to have abnormal FS after HT. Nevertheless, overall FS after HT appears to be excellent. These present-day findings may be helpful to clinicians who are tasked with counseling families regarding the expectations for post-HT life, and perhaps to critical care providers whose direct experience looking after the sickest patients may not always allow them to appreciate the long-term promise of HT therapy. Lastly, as patient survival continues to improve across eras, greater attention needs to be placed on understanding how to optimize the overall quality of life of children after a life-saving heart transplant and into adulthood.
