Obesity is associated with increased risk of atelectasis and impaired respiratory function during general anesthesia. An approach using an intraoperative high level of PEEP and alveolar recruitment maneuvers prevented those alterations, and has been proposed for routine intraoperative mechanical ventilation in obese patients. Whether this approach improves postoperative outcomes remains uncertain. The Protective Intraoperative Ventilation With Higher Versus Lower Levels of Positive End-Expiratory Pressure in Obese Patients (PROBESE) trial was conducted to test whether an intraoperative mechanical ventilation strategy with a higher level of PEEP and alveolar recruitment maneuvers reduces the incidence of postoperative pulmonary complications during the initial 5 postoperative days compared with a lower level of PEEP without alveolar recruitment maneuvers in obese patients undergoing surgery who are at increased risk for these complications.
